The David Hoose Commissioning Grant Application Uploads 2024

This form is where you attach the required files for your David Hoose Commissioning grant application. You must attach the following:

  1. Project Budget (using THIS TEMPLATE)
  2. IRS federal tax-exempt non-profit 501(c)3 certification letter (NOT a state certificate)
  3. Organizational Financial Report (IRS form 990, 990-EZ, or simple summary budget of expenses and revenue) (NOT IRS form 990-N)

You can also attach up to five files as "evidence of past successes" and "evidence of past composer successes" below (audio or video recordings, reviews, concert programs).

Your application must include a copy of the IRS letter (Federal, not state!) certifying that the applicant is a 501(c)(3) tax-exempt organization.
Files must be less than 20 MB.
Allowed file types: gif jpg jpeg png tif pdf.
Financial Report
Please upload a copy of your IRS form 990, 990-EZ, or a simple summary budget of expenses and revenue (NOT IRS form 990-N).
Files must be less than 100 MB.
Allowed file types: gif jpg jpeg png tif pdf doc docx odt.

Evidence of Past Successes

As part of this application, you should submit evidence of past successes, which may include up to 5 recent audio recording files, links to audio or video recordings, or other materials that demonstrate the organization's capacity to carry out the proposed project (concert programs, reviews, etc.). Recordings need not include complete works or movements, and should be limited to a total duration of 15 minutes or less. We accept audio files in common formats (preferably MP3, WAV, or AIFF). In addition, you may upload PDF files for programs, reviews, or other materials that might support your application. To submit a link instead of uploading a file, just include the link in the text of the Description, then you won't need to upload a file.
