Fill out the online application form below. All applications must be made using the online form; if this is a hardship for your organization, please contact us at firstname.lastname@example.org. Click on each section head to show or hide that section, or click the button.
Start by entering your organization's name. If you are applying on behalf of a chorus in our Chorus Directory, you can select the name from the list in the box below to fill in the Applicant Information fields using the information in the directory.
To pause and save your work, click the Save Form button at the bottom of the page. Make note of the application number displayed on the page that follows. You will receive an email acknowledgment the first time an application is saved, whenever the email address is changed, and when the application is successfully submitted.
To retrieve the form and resume work, return here and enter both the application number and applicant name in the box in the upper right of this page, then click Look up. (Or use the link from your acknowledgment email.)
When you have completed the form, click the Submit Form button at the bottom of the page. The form will be checked for completeness and then submitted for review by the committee. The contact person named on the application will be sent an email acknowledgment. A full copy of the completed application can be printed at any time from the link.
