Fill out the online application form below. Beginning this year, all applications must be made using the online form. If this is a hardship for your organization, please contact us via email: firstname.lastname@example.org, or by phone: 781-721-SING. Click on each section head to show or hide that section, or click the button.
Start by entering your organization's name. If you are applying on behalf of a chorus in our Chorus Directory, you can select the name from the list to fill in the Applicant Information fields using the information in the directory.
To pause and save your work, click the Save Form button at the bottom of the page. Make note of the application number displayed on the page that follows. You will receive an email acknowledgment the first time an application is saved, whenever the email address is changed, and when the application is successfully submitted.
To retrieve the form and resume work, return to this page (http://choralarts-newengland.org/application2017.html) and enter both the application number and applicant name in the box in the upper right of the page, then click Look up. (After the first lookup, you can double-click in the Number or Applicant fields to fill in these fields.)
When you have completed the form, click the Submit Form button at the bottom of the page. The form will be checked for completeness and then submitted for review by the committee. The contact person named on the application will be sent an email acknowledgment, and, later, a PDF copy of the application.
